‘How many sessions for back pain?’ is a budget question wearing a clinical coat. Fair. A family in Sahibabad or Crossing Republik needs to know if they are buying three visits or a season. What I will not do is type ‘twelve’ because a poster somewhere said twelve.
Session count follows irritability, how long the pattern has been rehearsing, whether the leg is involved, and whether your week can actually change — the Noida sit, the sofa-laptop, the water jar. Two people with ‘lower back pain’ on a form are not the same product.
Visit one exists to make the next number honest. First clinic session ₹199, first home session ₹499: assessment plus that day’s treatment, not a token that unlocks a secret pack. After that, I should be able to say ‘review in five to seven days’ or ‘if sitting minutes have not moved in two weeks, we change the plan’.
If a centre quotes a lifetime bundle before you have stood from a chair, you are buying their occupancy, not your back.
Acute mechanical back pain that started this week, red flags clear, you can still walk: often a handful of sessions plus a home programme, not a quarter of the year. The job is to restore confidence in sit-to-stand and a sit duration that matches work, then step back.

Home versus clinic does not magically change the count. Home costs more per visit because of travel. It can reduce total visits if the real driver is your chair and we coach it on day one. Clinic can be fewer visits if you need table space and you will actually travel to Sector 1.
End of visit one: ‘Your sitting lights this up after X minutes. This week, stand at Y, do these two movements, avoid that lift. Come back on date Z or message if the leg goes numb.’ That is a session plan. ‘See you till further notice’ is not.
End of visit three: we should look at the sit-minute number and the fear of bending. If both are stuck, we change the plan or we ask whether imaging or an ortho opinion is the missing piece — not add five more of the same.
You can book the first session without knowing the final count. You should not book visit eight without knowing why visit seven existed.
Bring your calendar. A plan that needs four morning slots you do not have will fail and then look like ‘physiotherapy did not work’. We design around Ghaziabad traffic, not around an imaginary retired week.
